Optometrist report, Royal Blind Society functional Vision Assessment, and Access Request and Disability Confirmation forms must be completed.
What happens after referral?
Forms are presented at DEC placement panel committee meeting.
Services Provided
• Identification of the educational implications of the student’s vision impairment in order to set educational goals.
• Provision of advice regarding any particular physical or environmental problems that may affect the student’s placement and program.
• Training in the use of adaptive technology
• Provision of alternate formats eg Braille
• Orientation and mobility within the school

• have an audiogram completed by Australian Hearing.
Referral
Children can only be referred through Australian Hearing after an audiogram test has been completed.
What happens after referral?
Language, speech and listening assessments are carried out to determine the child’s needs.

• In class support, one on one support and small group support to develop listening, speech and language skills
• Interpreter Service
• Note taking in the classroom
• Team teaching

Referral
An individual may self refer or may be referred by someone else as long as consent is given. You may apply by filling out the online application form available from www.guidedogs.com.au or by calling the office number listed above.
What happens after referral?
The client will be contacted within 10 days and an appointment will be made.
Services Provided
Children’s services:
• Guide Dogs Association provides children with Orientation and Mobility training which is tailored to the child’s individual needs, age and skill level. The training program involves areas such as pre-cane skills, cane training, bus travel, and use of residual vision and other senses. Instructors work with everyone involved to reinforce this training – at home, pre-school, school and in the community.
• Information and support is provided for families, childcare centers, schools and other agencies.
• Recommendations may be provided for environmental changes to school grounds and playgrounds.

Target Population
Children under school age, diagnosed with hearing and/or vision loss.
Eligibility
The early learning program is available to any child who:
• has a significant hearing or vision impairment, and
• is under the age of six, and
• is not attending school (preschool or day-care excluded)
Documentation of the vision or hearing impairment is necessary before entry to the program.

Services Provided
The Early Learning Program will:
• Develop an individualised program responsive to your child's needs and your family's priorities
• Provide you with information and support about hearing or vision impairment, and how it may impact on your child's development
• Help you to understand the unique needs of your child
• Provide regular input into your child's development
• Help you develop skills that support your child
• Help you obtain information about how and where to access other resources for your child
• Provide opportunities to participate in parent information sessions (via teleconferencing or videoconferencing)
• Encourage and support your occasional visits to North Rocks campus in Sydney, where there are opportunities for assessment and consultation with specialists such as an orthoptist, speech pathologist, adaptive technology consultant, educational psychologist, audiologist or cochlear implant co-ordinator.
• Work in collaboration with your child’s preschool or day care centre if required.

Referrals to Royal Blind Society can be made by parents, or by professionals involved with the family with parental consent.
What happens after referral?
Details are sent to a case manager, who contacts the client regarding an appointment and relevant services.

– multidisciplinary evaluations with an orthoptist, occupational therapist and educators specialising information technology for vision impairment
• Specialised information and advice, including
