Target Population

Speech Pathology
Children aged 0-12 yrs with speech/language difficulties.
Physiotherapy
Children 0-18 years with delays in motor development and physical disabilities (focus on Early Intervention).
Occupational Therapy
Children 0-12 with mild-moderate learning difficulties, 0-18 with physical disabilities (focus on Early Intervention).

Eligibility
Children who reside in New South Wales are eligible for this service, unless the child fits the eligibility criteria for DADHC services.

Referral
Referrals are accepted through letter, by telephone, or by dropping-in to the ACHS. Referrals can be accepted from parents/carers, paediatricians, child & family health nurses, teachers, general practitioners, other health professionals.
Please note referrals can only be accepted with parental/carer consent.

What happens after referral?
Parent or guardian contacted, and time scheduled for assessment (some services vary).

The Albury Community Health Service offers Speech Pathology, Physiotherapy and Occupational Therapy. Each service offers different types of intervention for the client and their carers.

Albury Community Health
Mental Health Service

Child and Adolescent Mental Health Service

Target Population
Includes children 0-18 years of age.

Eligibility
To be eligible a child must
• have serious behavioural disturbances, or
• have mental illness, or
• have a parent with mental illness.

Referral
A child’s family can self refer or a child can be referred by someone else. To make a referral contact the telephone triage service (‘Accessline’) above.

What happens after referral?
Referral information is presented during a weekly intake meeting and staff members decide if the child is appropriate for the service. The family is then contacted and told whether or not the child is eligible. If the child is eligible the family is either offered an appointment or put on a waiting list.

Family & Community Services: Ageing, Disability & Home Care (ADHC)

Target Population
Includes children aged 0-6 years with global developmental delay.

Eligibility
Children under six years of age are considered eligible if:
• the child demonstrates features that are consistent with a developmental delay or;
• the child demonstrates specific areas of delay in at least two areas of functioning. These delays must be determined from a norm referenced developmental assessment.

Referral
New referrals to ADHC services can be made by contacting intake on Toll free: 1300134450.
Referrals can be made by the family or by a service provider.

What happens after referral?
Once eligibility for service is determined an assessment of needs may be completed from which service provision is prioritised according to need and availability of services.

South West Brain Injury Rehabilitation Service

Target Population
Children with acquired brain injury (ABI)

Eligibility
School aged children with an ABI (i.e. not congenital or arising from birth injuries.) Injuries are typically traumatic and acquired after the first two years of life e.g. Motor Vehicle Accidents, falls, sporting injuries etc

Referral
Families can refer directly or referrals can be made by GPs, teachers etc. Documentation relating to accident (e.g. reports, scans etc) is required to determine eligibility of client.

What happens after referral?
If eligible an Initial Assessment is carried out. Subsequent assessments may include occupational therapy, speech therapy and/ or neuropsychology. These are followed up by collaborative development of an individual rehabilitation/community reintegration plan.

Target Population
Includes children aged 0 – 18 with Cerebral Palsy

Eligibility
Children 0 to 18 with Cerebral Palsy. Children must have significant and complex needs.

Referral
Made by parents only.

What happens after referral?
The Spastic Centre offers an Episode Based therapy.
Therapist will contact the client to make an appointment to visit at home and/or school to do an Individual Family Service Plan (IFSP). After this is drawn up to the satisfaction of the family and therapists and all the identified needs are met after the designated timeframe, the client will exit from the service. The client can enter the service again at any time for the identified needs or further needs.
