Target Population
Includes children 0-5 with disabilities.

Eligibility
• The child must live with a parent or carer.
• The child must have a diagnosed intellectual or physical disability or, in some cases a diagnosed medical condition.

Service Provided
This service provides 24 hour emergency respite for carers.

Referral
Self referral or referral through another service provider.
When emergency respite is required, the carer/service provider can ring the above 24 hour number and Carer Respite Centre staff will organise an appropriate respite service.

Cost of Service
There is no charge for this service.

Target Population
Young people with a disability

Eligibility
Young person must –
• live in Albury or local government areas of Greater Hume or Upper Murray
• be primarily in the care of their families
• be unable to independently perform the tasks of daily living without support or supervision
• have a physical and/or intellectual disability, a sensory impairment or a developmental delay

Referral Procedure
People can contact Family Link directly or be referred by anyone, as long as the child’s parents have approved the referral.

What happens after referral?
A Family Link Assessment is arranged with the child’s parents within 5 days of referral. The assessment will show whether the child is eligible, i.e. fits into funding criteria. If not the family is referred by Family Link to other more suitable services. If the child is eligible an individual respite plan is developed to meet the needs of both the child and the family. The start of respite depends on the availability of a suitable carer who is acceptable to the family.

• in home
• host family
• community access care
• special care through partnerships with other services
• other flexible respite options

Monday to Friday 9:00am – 4:30pm

Cost of Service
In line with Home and Community Care policy Family Link has a family contribution fee for respite received.
Difficulties meeting fees can be discussed confidentially with the Family Link Coordinator.


 

Mercy Centre Lavington

Target Population
School age children and children below school age, with intellectual disabilities.

Eligibility
To be eligible children must have an intellectual disability, and may have a physical disability or mental health issues on top of the intellectual disability.

Referral
Apply in writing to the Mercy Centre.

What happens after referral?
Referrals are dealt with on a most in need basis. This is decided by a panel when vacancies occur.

Services Provided
• Residential Accommodation/Respite on weekdays during the school term for children who attend school.
• Some weekend respite is available for children of any age.
• Case Management – Individual programmes are devised for all children to suit their individual needs and the needs of their families.
• Behaviour management services – development of behaviour management program.
• Family therapy.
• Support for siblings.

Office hours Monday to Friday 9:00am – 5:00pm
